 All new patients must complete and sign form #1.  You may complete and print the form at home or come 15 minutes before your first appointment to complete the form in the office. 

 Form #2 should be completed by each parent and by a school teacher if you are bringing in a child for evaluation of a behavior or learning problem. 

 Form #3 should be completed by all adult patients seeking evaluation for Adult ADHD. 

 Forms #4 and #5 should be completed by all adults seeking evaluation for depression, bipolar disorder, or other mood problems.
